What is the London Care Record?
The London Care Record is a secure view of your health and care information.
It lets health and care professionals involved in your care see important details about your health when and where they need them.
It can show doctors, nurses and other care professionals any conditions you have, your test results, medicines you take, anything you’re allergic to, plans for your care and other important information.
Having a single, secure view of your information helps speed up communication between care professionals across London, and beyond and helps informs the decisions they make about your care.
This helps to improve the safety of care and can save lives.

Who is part of the London Care Record?
Most GP surgeries and hospitals in London can now use the London Care Record and also share information into it.
Some local authorities, community health and care services, out of hours, the London Ambulance Service and other NHS 111 and mental health services are also able to use the London Care Record.
As some people living outside London receive specialist treatment in the Capital our partners in Milton Keynes and Hertfordshire and West Essex are also part of the London Care Record. We are currently considering what other neighbouring areas should also be included.

To maximise the London Care Record’s positive impact on health and care we continue to increase both the number of staff who can use it as well as those organisations sharing information into it.
How does the London Care Record work?
Each of London’s five Integrated Care Systems, in addition to Hertfordshire and West Essex and Milton Keynes, have their own systems for sharing data within their own local areas:
The London Care Record brings the information in these systems together securely and allows different health and care providers from across London and its surrounding areas to view it.
This means it can be seen by members of your health and care teams who are providing you with care and treatment in the region including doctors, nurses and other care professionals such as social workers, health assistants and therapists.
Only health and care staff directly involved in your care can view your information. Your local health and care teams monitor and log access to the London Care Record to make sure that it is being used appropriately.
The following graphic shows how information from across London and neighbouring areas is joined up through the London Care Record. It is based on a product called the Health Information Exchange (or HIE) from Cerner UK. A central Health Information Exchange is connected to a series of local Health Information Exchanges enabling data to be shared and accessed by health and care professionals across London.
What difference is the London Care Record making?
In total the London Care Record has been used well over 30 million times by health and care staff – with around one and a half million views each month by over 80,000 different health and care staff.
That is 30 million moments of care that have been supported by staff having the information they need when they need it helping them provide faster, safer and more effective care.
It has helped to:
Avoid patients having to repeat their medical histories
Prescribe medicines safely and accurately
Avoid unnecessary tests and referrals
Reduce unnecessary admissions to hospital
And so much more
