Hospital Pharmacists are using the shared care record to check what medication a patient is currently taking.
If someone presents at A&E and are asked if they are on any medication, they might say ‘I’m taking this and this, and something else for my blood pressure’, or ‘I’m taking this red and yellow pill but I don’t know what it’s called’. Hospital Pharmacists can now click through and see the information from the GP record detailing what the patient has been prescribed.
